IMPORTANT: To begin the approval process, please only apply if your safety rating is listed as “Satisfactory” or “None”.

Below, please provide the required criteria for qualification verification. All of required criteria must be submitted for this process. Once the required criteria has been received, please also be prepared to submit additional required documentation to be disclosed by our onboarding team including the following:

• Request your insurance carrier submit a Certificate of Insurance
  which must include $1,000,000 in Auto Coverage and $100,000
  in Cargo Coverage.
